Tubal ligation is a permanent method of female sterilization, which involves the tying up of the fallopian tubes, which obstructs the subsequent travel of the ovum from the ovaries to the uterus.

Annually, more than half a million women in the United States undergo tubal ligation, with the intent of limiting their family size.

Tubal ligation reversal is considered by many women (about six percent) with the intent to relive their experience of pregnancy and child birth. Tubal ligation reversal is referred to as ‘Tubal Re-anastamosis’.

Tubal ligation reversal procedures are available abroad at less than half the cost. International destinations where a tubal ligation reversal is conducted are Mexico, Costa Rica, India, Jordan, and Turkey.

Why do some women consider Tubal Ligation Reversal?

In addition to the renewed desire to mother a baby, many women consider tubal ligation reversal to overcome the undesirable effects of Post Tubal Ligation Syndrome.

The symptoms may include menstrual irregularities, loss of libido, vaginal dryness and early onset of menopausal symptoms (hot flushes, mood swings, irritability, palpitation, and anxiety).

However not all women with tubal ligation, can consider a reversal. Reversal depends upon certain factors like age, the procedure used for the tubal ligation, the exact position of the ligation, and fallopian tube length available after the procedure.

The medical team would require examining other health considerations before suggesting the viability of a tubal ligation reversal.

Success Rate of Tubal Ligation Reversal

Clinical evidence suggests that the success rate for tubal ligation reversal is about 20% to 80%. The most important consideration is the age of the women, with women below forty years of age showing pregnancy success rate of as high as 80%.

Furthermore, the technique used for re-canalization of the fallopian tubes also plays a crucial role. Conventional methods of reversal have shown a success rate of 70% while, the use of microsurgery has shown slightly higher success rates of up to 90% (i.e. 90% of women get pregnant within a year of the reversal surgery).

Preparation for a Tubal Ligation Reversal

There are several issues that need to be considered before undertaking the Tubal Ligation Reversal Surgery. These parameters are to be considered in the following sequence to verify the need and rationale for undertaking the tubal recanalization surgery.

The analysis of the sperm quality of the male partner is an essential pre-requisite. If the quality is good, re-canalization of the tubes is an option. If the quality of the sperms is poor, IVF is a better option.

The status of the fallopian tubes is an essential consideration. The tube needs to be long enough (at least three to four inches long).

The two stumps should not have a narrow diameter and should be almost similar in size. This helps easy reunion of the two stumps increasing the chances of a pregnancy.

A pre-existing pelvic condition like endometriosis or scar tissue reduces the chances of conception after a tubal ligation. This needs to be reviewed accurately.

For this the patient is made to undergo a hysterosalpingography to assess the patency of the tubes. However a Laparoscopy is preferred as it can assess and detect any pelvic disease more accurately.

The analysis of the quality and quantity of the ovum produced by the ovaries is also required. It is critical to understand if fertilization is possible through the normal way. If not, In Vitro Fertilization (IVF) is a preferred option.

Tubal Ligation Reversal Surgery: The Procedure

There are several ways and techniques of conducting a Tubal Ligation Reversal Surgery, however, broadly, the procedures can be classified in the following two categories:

Conventional Reversal Surgery

A major surgery which takes a couple of hours and is conducted under general anesthesia. The surgeon will unclamp and remove any device which is blocking the tubes and reattach the tubes by placing an implant or sampling, stitching together the two ends.

This procedure requires hospital stay of about five days and it will take about a couple of weeks for complete recovery. The complications include bleeding, infection and a risk of damage to the surrounding organs.

Microsurgery

Unlike the conventional procedure, microsurgery reversal takes less than an hour and can be conducted on an outpatient basis.

The advantages of this procedure are that it minimizes the complications of surgery like bleeding and pain and also considerably reduces hospitalization stay.

The greatest benefit of the microsurgery is that it allows the tiny inner openings of the tube to be connected accurately thereby reducing chances of error or failure in conception.

Some Useful Information

  • The risk of developing an ectopic pregnancy after the reconstructive surgery ranges between 4 to 10%.
  • In Vitro Fertilization (IVF) is a better option in cases of older women, and in women with significant tubal or pelvic pathology.
  • There is no significant difference in terms of the cost comparison between the two procedures. Even the success rates of pregnancy after a re-canalization surgery is about 40-75% which is comparable to IVF pregnancy success rate of 60-70%. Hence the decision of IVF versus re-canalization depends upon the patient’s condition rather than on economic or success rate factors
